I second the Gummo vote. Especially the opening scene with the kids and the cats. Hmmm.
For me, without a doubt it's Dr. Caligari (1990). Strange beyond belief.
EVEN DWARFS STARTED SMALL (1970) was pretty nutty...and I didn't see anyone mention it earlier, although I didn't read each and every post. One of Herzog's first films, and the one that put him in the spotlight.
Although I really liked the film, I thought Irreversible was pretty F***ed up. Very effective filmmaking, left me breathless. It was literally one of those films that -- after viewing it, I was left mumbling, "That was f***ed up."
Requiem for a Dream.
I've seen Cannibal Holocaust, Ichi, Audition, and Nekromantic and while they are all beautifully fucked up in their own special way, none of them messed with my head the way Requiem did.

Trouble Every Day was a big disappointment coming from Claire Denis & Vince Gallo, just boring
Destricted(a film consisting of short porn films) saw it at Sundance, I wouldn't want to sit through "Hoist" directed by Matthew Barney, or the film directed by Gaspar Noe,
The Gift, (the film directed by Perry Farrel & I believe Casey Niccoli)
Satyricon, tried to watch around 10 years ago, couldn't sit through it...it freaked me out
New York Ripper, I like Fulci but this film was just too sleazy, left me feeling depressed
Requiem for a Dream hands down.
Being a fan of them, I just had to say the director of Spun used to be the drummer of Bathory

A few off the top of my head that Ive seen
I'm going to go in a different direction and throw out the 10 or so minute scene in Lynch's "Blue Velvet" where we meet Frank (Hopper) for the first time. That scene alone stands as one of the more disturbing moments in film for me. "Dont you fucking look at me!!!"
Watched Bas Moi for the first time recently, not really fucked up as a whole, but there were some certain scenes that were pretty fucked up.
August Underground bored the shit out of me, Cannibal Holocaust is a classic but in this day and age it ranks as less-horrific than some of the shit you see on ogrish.com. I agree with all those about Requeim for a Dream, it is brilliant, and of course the dead baby in the crib in Trainspotting gets to me everytime.
Any of the Guinnea Pig films are all fucked up, especially Flower, Flesh and Blood.
However....I would have to say, just by readin the basis of it, the "Ultimate Atrocity" is undoubtedly the most fucked up thing anyone will ever see.....assuming it is indeed on film.
