Scrapbook is probably the most disturbing movie I've seen in a long, long time.
Graphic Sexual Horror
I wanted to mention a documentary that horror fans might find interesting, as it's subject, INSEX.com has heavily influenced most contemporary sexual horror creators/directors / visual artists.
Graphic Sexual Horror is a documentary about INSEX.com, the most notorious bondage website.
The correlation to horror movies? Aplenty. Saw, for example, has been heavily influenced by the work of PD, aka Brent Scott.

And the imagery in the documentary is probably the most horrible sexual you will ever see.
First, it's real, second, it's so explicit that the film cannot be shown in Great Britain (instant Jail).
It is beyond R or NC-17 rating.
Hmmm . . . most graphic? Most of the ones that come to mind have already been mentioned, but I'll toss a few more out:
Shivers/They Came From Within (not really much overt sex, but the sexual overtones are way out in the open)
Would the worm rape scene in Galaxy of Terror count?
The hardcore scenes in this film were inserted later and are not the actual actors.
Has anyone else watched the rather boring Amateur Porn Star Killer films? Part 3 is actually being released soon.
The August Underground films, especially Penance.
I recall Don't Answer the Phone being pretty sexually graphic in parts. Last House on the Left also had several sexually graphic parts as well.
I agree.Jose Larraz's VAMPYRES is quite erotic,almost dreamlike, and tends to mix it's eroticism with bloody mayhem as an odd counterpoint.Lovely ladies here(and a lot of blood).
Although not horror, the ending of "To Be Twenty" is one of the most shocking scenes I've ever seen. xploitedcinema.com used to carry this film. Glad I picked it up before they stopped. The whole movie works as a sex comedy, except for the final rape scene. Couldn't get that scene out of my head for days.
"The Untold Story" has a pretty brutal rape-by-chopsticks scene. As does "Man Bites Dog" (rape minus chopsticks).
I agree with the nomination of Shivers / They Came from Within. Another real dirty in the 'bad way' kinda movie. Leaves one yearning to take a shower directly after viewing.
I would also nominate Street Trash. Its' depiction of women and sexuality will really leave a bad taste in one's mouth.

That was a gross scene. The movie is pretty good but I truly could have done without that nasty thing.
It's so cartoonish, I couldn't see that as something bothering to get 'up in arms' over. Plus, I always saw in that movie there needing to be some open cut or scratch-contact necessary to turn the people into demons. That was basically the only moment when she was vulnerable to being transformed by the force outside the cabin. Unless you count the scratching of the vines as she ran.
Yeah, seriously. House on the Edge of the Park is garbage. Not worth anyone's time. (Quite a ridiculous movie, as well.)
It's worth my time. Nobody plays a sleaze-bag quite like David Hess. Giovanni Lombardo Radice is quite funny as his semi-retarded sidekick. Sure the film is mean spirited and misogynistic, but because it is ridiculous as you say, it can't be taken seriously and can provide some trashy Italian film fun.
I'll throw the Tombs of the Blind Dead series into the mix.
Erotic Nights of the living dead, is most explicit.
DarkWolf (2003) had some excessive erotica happening. Not really explicit but quite eye opening.
Lady Stay Dead (1981) was borderline explicit with a huge amount of sleaze.
Even though comedy but I thought the nudity and eroticisms in The Invisible Maniac (1990) was kind of "pushing it". Actually, saucy scenes had to be trimmed for an R rating :eek1:
The Entity- No nudity, but the theme kinda qualifies.

Malabimba, it's awesome too. It's like Burial Ground if Burial Ground was even fucking crazier and had porn.

Toolbox Murders couldn't really be considered a sexually explicit horror film unless just containing some nudity is all that is required.
If you want extreme and don't mind animation, head East young man. I haven't seen it in decades, (and I am sure it has been surpassed since) but "Legend of the Overfiend" was the top tier of the anime tentacle-porn and was really extremely unsettling in it's constant barrage of women being raped by giant tentacled monsters. There's a whole subgenre in anime of these things.
